4. PROFESSIONAL PSYCHOLOGIST PROGRAM

• Registration fee: IDR 500,000
• Color scan of Bachelor’s Diploma and Transcript in Psychology
• Undergraduate Program accredited B or higher, minimum GPA 3.00
• Curriculum Vitae (CV)
• Passport photo (soft copy)
• Color scan of National Identity Card (KTP)
• Motivation Letter: narrative format, min. 10 lines, no title; handwritten on A4 blank sheet using ballpoint, signed, scanned in PDF.

7. MASTER OF LAW

• Academic Transcript (GPA = 3.0 PTN; = 3.5 PTS)

9. FAMILY MEDICINE SPECIALIST – Sp. KKLP

• Photo/Scan of UKMPPD Pass Certificate
• Photo/Scan of valid STR and SIP
• Photo/Scan of Physical & Mental Health Certificate
• Photo/Scan of legalized MBBS/MD Diploma & Transcript (GPA = 2.75)
• Study Permit from employing institution
• Letter confirming = 1 year clinical practice
• Updated CV
• Recommendation Letter from PERDHAKI (members only)
• MMPI Test Result (valid = 1 year)
