1. MPSI SAINS

-       A scan or photo of Academic Transcript

-       A scan or photo of Bachelor’s Degree Diploma/Letter of Graduation

-       A scan or photo of ID Card

-       A colored passport photo

-       Master of Psychology Admission Form (Download File)

-       Personal Data (Download File)

-       Motivation Letter (Download File)

-       Statement of Purpose (Download File)

-       2 (two) Reference Letters from referrers with no familial relationship (Download File)

 

2. MLTBI, MM, MBIOTEK, MTM, MTE, MAB, MET, MAKSI, PPAK & PPI

-       A scan or photo of Bachelor’s Degree Diploma (Applicants who graduate overseas are required to enclose the diploma with its equivalency certificate as issued by DIKTI)

-       A scan or photo of Bachelor’s Degree Academic Transcript

-       Passport Photo 3x4

-       A scan or photo of ID Card

-       CV (Curriculum Vitae)

-       A recommendation letter from supervisors or academic advisors/lecturers with the following format:(Download File) (not compulsory for MLTBI program)

 

3. MBIOMED

-       A scan or photo of Bachelor’s Degree Diploma (Applicants who graduate overseas are required to enclose the diploma with its equivalency certificate as issued by DIKTI)

-       A scan or photo of Bachelor’s Degree Academic Transcript

-       Passport Photo 3x4

-       A scan or photo of ID Card

-       CV with work details

-       Recommendation letter from supervisors or academic advisors/lecturers with the following format:  (Download File)

4. PSYCHOLOGY PROFESSION 

-  Minimum GPA of 3.00 (below 3.00 GPA will not be able to register)

-  Diplomas and transcripts must have a minimum program accreditation of B

-  CV

-  Motivation letter with conditions: Written in narrative form, at least 10 lines without title, the theme of the writing is the reason why you want to study professional psychology. The motivation letter ends with a signature. The motivation letter can be written in ballpoint pen on A4 size portrait HVS paper and then scanned and formatted in PDF form.

- Two (2) recommendations from referees who have no family relationship.

5. DLTBI

-       A scan or photo of Bachelor’s and Master’s Degree Diploma (Applicants who graduate overseas are required to enclose the diploma with its equivalency certificate as issued by DIKTI)

-       A scan or photo of Academic Transcript (Bachelor’s and Master’s Degree)

-       Colored Passport Photos

-       A scan or photo of ID Card

-       CV with work details and personal details

 

6. DPSI

-       Motivation Letter (Download File)

-       Referral Letters from 2 (two) persons, one of whom is Academic/Thesis Advisor or Supervisor (Download File)

-       A copy of Bachelor’s and Master’s Degree Diploma

-       A copy of Bachelor’s and Master’s Degree Academic Transcript, GPA >3.26

-       A copy of ID Card

-       Colored Passport Photos 3x4

-       A research plan for dissertation max. 5 pages

-       A sample of academic writing: articles / research reports / books

-       Resume / Curriculum Vitae Curriculum Vitae (CV should include personal information and details concerning academic publications (conferences/journal articles), professional work (part-time, research, etc.) and memberships in a professional organization)

7. MAGISTER OF LAW

- Scan / Photocopy of S1 diploma (Candidates with diplomas from abroad please attach diplomas that have been equalized by DIKTI)

- Scan / Photo Transcript (S1)

- 3x4 photo pass

- Photo of ID card

- Letter of recommendation from superiors or lecturers / mentors in the following format: (Download File)

- Certification in the field of law / Certificate of working in the field of law (specifically for S1 origin outside of Law) 

- Motivation Letter

8. PHARMACIST PROFESSION

- Scan / Photo of S1 / SKL diploma of Bachelor of Pharmacy graduates

- Scan / Photo of Recommendation Letter from the University at least the Head of Study Program (specifically for New Student Candidates Non-Alumni of Unika Atma Jaya) (Download File)

- Scan / Photo of Accreditation of Undergraduate Origin Study Program. Minimum “Excellent” 

- Scan / Photo Transcript of grades (minimum undergraduate GPA is greater than 3 PTN, 3.5 PTS)

- 3x4 color photo

- CV

- Drug-free certificate (Download File)

- Health certificate from a general practitioner

- MMPI Test Results, may be psychologists and psychiatrists (maximum of the last 1 year)

9. PRIMARY SERVICE FAMILY COOKING SPECIALIST PRODI (Sp. KKLP)

- Photo/Scan of UKMPPD Graduation

- Photo/Scan of Active STR and SIP

- Photo/Scan of Physical and Spiritual Health Certificate from Puskesmas or Hospital

- Photo/Scan of legalized diplomas and transcripts of S.Ked and General Practitioner “GPA of General Practitioner Profession is approximately (>_) 2.75

- Certificate from the Head of Institution regarding study permit * for those who work in government and private institutions

- Certificate from the head of the institution regarding practical experience of at least 1 year

- Latest CV

- Letter of Recommendation from PERDHAKI *especially for member participants

For registrants of Odd Semester

Administrative Requirements files are uploaded through join.atmajaya.ac.id

Administrative Documents shall be sent at the latest on September 7, 2024 and admission test is conducted on September 4, 2024.

The deadline for submission of Psychology Profession registration documents:
